FLY2AT Course for Professionals


Florida Department of Health, Children’s Medical Service welcomes you to our Florida Youth 2 Adult Transition (FLY2AT) course for Professionals!

This course was developed to introduce youth-to-adult transition concepts and resources to professionals who support youth and young adults, and their families, during their transition to adult systems of care. A special focus is provided to youth with chronic physical, developmental, behavioral, and/or mental health conditions as they typically utilize more and benefit greatly from transition services.

Learning Outcomes

Individuals will be able to:

  • Explain youth to adult transition concepts
  • Identify professional resources available for further exploration

This course contains four (4) learning modules. Each module includes major transition-related concepts and highlights current best practices. Each module also presents resources aligned with the overall topic of the module. The course navigates users from module 1 to module 4; however, the user may choose to experience the learning modules in any order they desire. Once the user completes all four modules, they will be awarded a certificate of completion, which can be used for one (1) service hour.
