Service Delivery Resources

• The Medical Home Portal website was initiated in 2001 to improve the care of children with chronic conditions and special health care needs, providing evidence-based clinical guidance for primary care of rare and common conditions, local and national resources for families and case managers to improve care, and support and information for families of children with complex care needs. Unexpected circumstances in 2024 necessitated the need to archive the expansive content through the University of Utah’s Spencer S. Eccles Health Sciences Digital Library in collaboration with the University of Utah’s Department of Pediatrics and remains available for access at The Medical Home Portal Archives.
